✓ If you are a current Boston IVF patient and wish to schedule a semen analysis or sperm cryopreservation appointment, please contact your Boston IVF doctor’s administrative assistant.

✓ For Boston IVF patients in New England, please call (888) 300-2483 and select Option 1.

✓ For Boston IVF patients in New York, please call (888) 300-9660 and select Option 2 within your specific NY location.

✓ If you are NOT a Boston IVF patient and wish to book a semen analysis, you must have your doctor (primary care, urologist, etc.) fax a lab order to your Boston IVF location:

✓ Please make sure the lab order contains your name, date of birth, and phone number, along with the requested testing.  

✓ Once this lab order is received, you will receive a call from Boston IVF to schedule your appointment. Please allow several days for processing.

✓ Please note: insurance is not accepted for non-Boston IVF patients. Payment is collected at the time of scheduling.
